We are only considering prospective patients permanently residing in the South Okanagan area who are not already attached to another provider. As part of our intake process, we verify whether a prospective patient is currently affiliated with an existing provider, in accordance with legislative changes to address shortages.
 

A separate form must be completed for each individual.  We try to accommodate (not prioritize) families, but we cannot guarantee that everyone will be accepted at the same time. 
 

Each adult must provide their own email address.  Children will be considered dependants of one parent patient and set up on the email address provided in this form.
 

Please refrain from contacting the clinic within three months of submission to inquire about your waiting list status. Submissions will be reviewed as capacity allows, and an email will be sent when we're ready to onboard you.  If you do not respond to the invitation in one week, we will move on to the next in line.
 

Submission of the form does not guarantee acceptance as a patient.
