Your past medical records can be transferred to us. In order for us to request it from your previous physician, you have to complete the form here and mail it to your previous physician's office. Alternatively, you can complete the form in our office and we can fax it to your previous physician's office.

* Note that your previous physician is allowed to charge you for the release of your records as this is not covered by BC Healthcare (MSP).
